One of the RH-series, and the one you read most often.
Ohio BWC’s vocational rehabilitation forms run in an RH series. The ones a case manager handles constantly:
- RH-42 — assessment report
- RH-44 / BWC-3012 — comprehensive rehabilitation plan
- RH-45 — authorization request
- RH-46 — progress report
- RH-10 — job-search contacts
The RH-10 is the injured worker’s record of employer contacts during a job search: where they applied, when, to whom, and the outcome.
Reviewing it is work, and work is billable.
“Rec’d/rev’d job-search logs” is a real activity with a real code — typically documentation review, W3220 at Level 1 — and six minutes of it is a tenth of an hour on the bill. Across a caseload of fifteen to forty injured workers, that is a meaningful line of a week’s billings that a counselor doing this by memory on Sunday simply forgets.
The RH-10 is also the document that answers a question about plan progress. When an MCO asks whether the job search is real, the contact log is the evidence — not your invoice.
