The level belongs to the case, not to the activity. Bill W3235 on a Level 2 case and the MCO pays the Level 1 rate, correctly, against what you sent — and nobody there will ring you about it. It is small enough to miss on one bill and large enough to matter across a year of them, which is precisely the shape of error a paper cheat sheet fixes and a database fixes better.
In the daily grid the case carries its level, so the suffix and the rate come with the code as you type. When BWC changes the schedule in October, the new rate ships as a data update — and work you already billed keeps the rate it was billed at, the way an audit expects.
