Ohio BWC’s service codes for vocational rehabilitation — W3207, W3220, W3230, W3235 and the rest — priced by rule and billed in tenths of an hour.
W-codes are the Ohio Bureau of Workers’ Compensation service codes for vocational rehabilitation. They ride on a CMS-1500 where a clinical biller would put CPT codes, and they are priced by rule — OAC 4123-18-09, updated every October.
The plan-implementation family is leveled. The bare code bills Level 1; the same code with W2 on the end bills Level 2; there is a third level above that:

Which means the wrong variant is not a coding preference, it is an underpayment — and it is one nobody at the MCO will correct for you. A W3235 billed on a Level 2 case is $2.50 an hour of your own money left on the table, every time.
The other half of the arithmetic is the unit: $81.90 is the hour, so six minutes is $8.19 and box 24G says 1.
