A court form, because Nebraska runs this through its court.
Nebraska’s vocational rehabilitation system sits with the Workers’ Compensation Court. Counselors are court-certified under NWCC Rule 40, and the paperwork — including the VR-44S billing form — is the court’s own.
That is a different center of gravity from a carrier-driven state: your standing comes from the court, and so do the forms.
Same claimant, two payers, depending on the phase.
The billing fact that catches people in Nebraska is not the form. It is who owes the money. Evaluation and plan development are billed to the employer or carrier. Plan implementation is paid by the state Trust Fund.
So one injured worker can generate two receivables, owed by two entirely different payers, at two points in the same case, with two different follow-up routes. Recorded as one row per claimant, the second one is the one that goes missing.
Nebraska also gives loss-of-earning-power evaluations a rebuttable presumption — see LOEP — which raises the value of a contemporaneous, specific record behind the bill.
